We are not designated as a PCP office however we are happy to provide the same services if you do not have one, or on the waitlist. This is covered by insurance!

• No insurance or not in network? We can do cash pay and/or superbill for reimbursement. 

A superbill is a detailed, itemized receipt to submit for reimbursement from the insurance company.

Most insurance companies have the form available on their website or app.

We provide an invoice containing information such as date of service, diagnosis codes (ICD-10), procedure codes (CPT), and charges, allowing the insurer to process the claim.  

Example:

Clinic Fee 

$150 (plus any testing if needed) is paid by the patient at the time of service.

Reimbursement Amount

80% of the "allowed amount" ($140), which equals $112. This is what your insurance will pay back to you.

Final Net Cost to Patient

The initial payment ($150) minus the insurance reimbursement ($112), for a net cost to the patient of $38 in this example.

Don't forget, you may still need to meet your deductible.
